In the last two decades, robot-aided rehabilitation has become widespread, particularly for upper limb movement rehabilitation. In this Doctoral Consortium I present a system for physical and cognitive rehabilitation that uses a combination of Serious Games to allow the monitoring and progress tracking of a person during physical therapy. The system records physical and cognitive states through the interaction with the advance robotic arm in order to assess the users hand-eye coordination, response interaction, working memory and concentration rates.

Rehabilitation for chronic conditions resulting from acute or progressive disease might be delivered in an outpatient facility as in the case of telerehabilitation, selfrehabilitation and, more generally, in the context of home-based rehabilitation to improve the patients’ quality of life.
Here we present the emerging field of home-based applications for continuous digital health, focusing in particular on low-cost rehabilitation systems for motor disabilities based on multimedia and gaming technologies. Innovative technologies for telerehabilitation are illustrated. We also present recent advances in telerehabilitation, considering the most relevant projects that best represent new trends for research and development of new technologies and applications in this context. Telerehabilitation (TRH) is the provision of rehabilitation services at a distance using information and communication technologies (ICT) . TRH services might be needed for diagnosis, assessment, consultations, and monitoring, as well as to supervise therapies or therapeutic settings or to propose interactive therapies. Rather than being described as a super specialization of rehabilitation, therefore, TRH should be viewed as an alternative way to deliver rehabilitation services, particularly suited for chronic conditions that might benefit from home-based care. It has been demonstrated, in fact, that improving motor function, a major goal of any rehabilitative treatment, should be pursued even in the chronic stages of disabling condition, like stroke, Parkinson’s disease, and multiple sclerosis, for which telemedicine could be seen as a great opportunity to allow remote diagnosis and clinical monitoring.

OBJECTIVE. We quantified the range of motion (ROM) required for eight upper-extremity activities of daily living (ADLs) in healthy participants.
METHOD. Fifteen right-handed participants completed several bimanual and unilateral basic ADLs while joint kinematics were monitored using a motion capture system. Peak motions of the pelvis, trunk, shoulder, elbow, and wrist were quantified for each task.
RESULTS. To complete all activities tested, participants needed a minimum ROM of −65°/0°/105° for humeral plane angle (horizontal abduction–adduction), 0°–108° for humeral elevation, −55°/0°/79° for humeral rotation, 0°–121° for elbow flexion, −53°/0°/13° for forearm rotation, −40°/0°/38° for wrist flexion–extension, and −28°/0°/38° for wrist ulnar–radial deviation. Peak trunk ROM was 23° lean, 32° axial rotation, and 59° flexion–extension.
CONCLUSION. Full upper-limb kinematics were calculated for several ADLs. This methodology can be used in future studies as a basis for developing normative databases of upper-extremity motions and evaluating pathology in populations.

Over recent decades, experimental and clinical stroke studies have identified a number of neurorestorative treatments that stimulate neural plasticity and promote functional recovery. In contrast to the acute stroke treatments thrombolysis and endovascular thrombectomy, neurorestorative treatments are still effective when initiated days after stroke onset, which makes them applicable to virtually all stroke patients. In this article, selected physical, pharmacological and cell-based neurorestorative therapies are discussed, with special emphasis on interventions that have already been transferred from the laboratory to the clinical setting. We explain molecular and structural processes that promote neural plasticity, discuss potential limitations of neurorestorative treatments, and offer a speculative viewpoint on how neurorestorative treatments will evolve.

Assisted motor therapies play a critical role in enhancing functional musculoskeletal recovery and neurological rehabilitation. Our long term goal is to assist and automate the performance of repetitive motor-therapy of the human lower limbs. Hence, in this paper, we examine the viability of a light-weight and reconfigurable hybrid (articulated-multibody and cable) robotic system for assisting lower-extremity rehabilitation and analyze its performance. A hybrid cable-actuated articulated multibody system is formed when multiple cables are attached from a ground-frame to various locations on an articulated-linkage based orthosis. Our efforts initially focus on developing an analysis and simulation framework for the kinematics and dynamics of the cable-driven lower limb orthosis. A Monte Carlo approach is employed to select configuration parameters including cuff sizes, cuff locations, and the position of fixed winches. The desired motions for the rehabilitative exercises are prescribed based upon motion patterns from a normative subject cohort. We examine the viability of using two controllers — a joint-space feedback linearized PD controller and a task-space force-control strategy — to realize trajectory- and path- tracking of the desired motions within a simulation environment. In particular, we examine performance in terms of (i) coordinated control of the redundant system; (ii) reducing internal stresses within the lower-extremity joints; and (iii) continued satisfaction of the unilateral cable-tension constraints throughout the workspace.
